Fecal Elastase Test

The Fecal Elastase test measures the amount of elastase, a digestive enzyme produced by the pancreas, present in a stool sample.

Elastase helps break down proteins in food, and its level in stool reflects how well the pancreas is functioning.

This test is used to assess for exocrine pancreatic insufficiency (EPI), a condition where the pancreas does not produce enough digestive enzymes, leading to problems with nutrient absorption.

Your healthcare provider may recommend a Fecal Elastase test if you or your child have symptoms suggestive of pancreatic insufficiency, such as:

  • Persistent diarrhea or loose, greasy, foul-smelling stools (steatorrhea)
  • Unexplained weight loss
  • Abdominal pain, bloating, or gas
  • Malnutrition or signs of poor nutrient absorption
  • Difficulty gaining weight or delayed growth in children

This test is also used to monitor pancreatic function in people with chronic pancreatitis, cystic fibrosis, pancreatic surgery, or other conditions affecting the pancreas167.

  • Collect a fresh stool sample (about 5 grams) in a clean, dry container.
  • Do not add any preservatives to the sample.
  • There is generally no need to stop taking pancreatic enzyme supplements before the test56.
  • If you cannot deliver the sample immediately, it can be refrigerated or frozen to preserve enzyme activity. Avoid samples contaminated with urine, blood, or excessive mucus56.

  • : 200–500 µg elastase/g stool
  • : 100–200 µg elastase/g stool
  • : Less than 100 µg elastase/g stool56

Reference ranges may vary slightly depending on the laboratory.

Results are typically available within 3 to 5 days after the laboratory receives your sample6.
