Test Description
Immunofluorescence analysis using Michel’s Medium is a diagnostic method used to detect immune system proteins in skin or mucosal tissue samples. It’s especially helpful for diagnosing autoimmune skin diseases, where the presence and location of certain antibodies provide vital clues.
Michel’s Medium serves as a preservation and transport solution for biopsy samples intended for Direct Immunofluorescence (DIF) testing. Unlike traditional fixatives, it maintains the structure and function of immune proteins by preventing their breakdown, allowing accurate testing even after several days at room temperature.
DIF uses fluorescent-labeled antibodies to visualize immune reactants like IgG, IgA, IgM, and C3 directly in the tissue. This technique is essential for diagnosing conditions such as pemphigus vulgaris, bullous pemphigoid, and linear IgA disease. Tissue preserved in Michel’s Medium provides results as reliable as those from fresh frozen samples, making it the preferred method for many diagnostic labs.
Indications of the Test

Suspected autoimmune blistering diseases, such as pemphigus vulgaris, bullous pemphigoid, epidermolysis bullosa acquisita, and linear IgA disease.

Evaluation of vasculitides and connective tissue diseases with cutaneous manifestations, such as lupus erythematosus.

Investigation of unexplained mucocutaneous lesions where immune-mediated pathology is suspected.

Patient Preparation/Instructions

No special preparation or fasting is required before the biopsy collection.

Inform your healthcare provider about any medications you are taking, especially immunosuppressants or corticosteroids, as these may affect the sensitivity of the test.

The biopsy should ideally be taken from normal-appearing skin adjacent to a lesion or from the lesion itself in the case of vasculitis or connective tissue disease.

Once collected, the tissue specimen is immediately placed in Michel’s Medium and kept at room temperature until it reaches the laboratory.

Specimens should be processed within five days to ensure optimal preservation of immune complexes and antigenicity
Typical Reference Values
Immunofluorescence analysis is qualitative and descriptive rather than quantitative. The results are interpreted by a pathologist and reported as follows:

Specimens transported in Michel’s Medium can be stored at room temperature for up to five days without loss of diagnostic integrity. Once received by the laboratory, processing and reporting typically take 2 to 5 working days, depending on laboratory workflow and the complexity of findings
