Transferrin Level Test

What is the Transferrin Level Test?

The transferrin level test is a blood test that measures the level of transferrin in your blood.

Transferrin is a protein made by the liver. Its main job is to transport iron throughout your body, helping red blood cells carry oxygen to the tissues.

This test helps your doctor understand how well your body can transport and use iron. It is commonly ordered alongside other iron tests (such as serum iron, ferritin, and TIBC) to get a clear picture of your iron status and to support the diagnosis of anemia or iron overload conditions.

Indications of the transferrin test

Your doctor may recommend a transferrin test if you have:

  • Symptoms of iron deficiency anemia (tiredness, weakness, pale skin, shortness of breath, dizziness, headaches)

  • Symptoms suggesting iron overload (joint pain, fatigue, unexplained weight loss, abdominal pain)

  • Chronic health conditions that could alter your iron levels (kidney disease, liver disease, cancer, chronic infection)

  • Ongoing monitoring of known anemia or hemochromatosis
This test helps diagnose the cause of anemia, iron deficiency, or iron overload, and can also monitor how you respond to iron therapy.

Patient Preparation/Instructions

  • Fasting: You may be asked to fast (no food or drink except water) for 8–12 hours before your blood sample is collected to ensure accurate results.

  • Medications: Inform your doctor about any medicines or supplements you are taking, as these can affect your test results.

  • Timing: It’s best to take the test in the morning, because iron levels can fluctuate throughout the day.

Typical Reference Values

Normal reference ranges vary by laboratory, but typical adult values are:

  • Transferrin: 200–400mg/dL

  • Transferrin saturation: 15–50%
Your lab report will show the specific reference range for you. Higher transferrin levels may mean iron-deficiency anemia, while lower levels may point to chronic disease, malnutrition, or liver concerns.
